This information applies only to those Carers in the Hinckley & Bosworth area, which covers the south west of Leicestershire from Ratby in the north to Twycross in the west.
Registration Form
The Registration Form is used to inform your GP and the Carers Project that you are a Carer and that you feel that you may need support at some time. You may print the form off and complete it as detailed and then give it to your GP. The Surgery will then record that you are a carer and ensure that you get an annual chance to discuss your well being with the surgery. The form will then be copied to the Project and you will receive a confirmatory letter and further details. Alternatively you may contact the Project on 01455 231998 to discuss registration
